[Montgomery County, MD] Kids Spot Registration Form

How to Complete the Kids Spot Registration Form:

1. Complete the necessary fields. Fields with a red asterisk (*) are required fields that need to be completed in order to submit the form.

2. Once you have completed all the necessary fields, click the Submit & Sign button in the bottom-right corner.

3. A Create Your Signature box will appear:

- Type in your full legal name and email address.

- Your initials will be auto generated based on what you type into the full legal name field.

- You can edit the signature if you'd like or you can leave it as the default.

- Check the I agree checkbox.

- Click the Apply Signature button.

4. Click the Next button in the bottom-right corner, then the Apply Signature button to apply your initials and signature to all the necessary fields. Repeat as many times as necessary.

5. Once all initials and signature fields have been completed, click the Finalize & Submit button in the bottom-right corner.
