I have watched the orientation video at www.rockvilleapp.com/startand understand the program rules.

I understand that I may be removed from a group session and counted absent if I do not follow the program rules.

I understand that if I miss more than 6 classes, for any reason, I will be required to start the program over.

I understand that the standard fee per session is $20, that I may apply for a fee reduction by submitting proof of income, and that I am responsible to pay $20 per session until I do so.
